Accountable Care Organization (ACO) Participation

AMG Senior Medical Group participates in Advanced Illness Partners (AIP), a network of independent healthcare providers collaborating to improve care quality and reduce healthcare costs. Through this participation:

  • We may share health information with AIP ACO and other participating providers for care coordination and quality improvement.
  • The ACO uses data analytics to support better patient outcomes.
  • Your medical information will only be used as permitted by HIPAA and applicable laws.

If you have concerns about sharing your information for ACO purposes, you may contact us to discuss your options.

Our Responsibilities

We are required by law to:

  • Maintain the privacy and security of your protected health information (PHI).
  • Notify you promptly if a breach occurs that may have compromised your PHI.
  • Provide you with a copy of this Notice upon request.
  • Follow the duties and privacy practices described in this Notice.
